29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about July 12, 2013, on the east side of the commercial construction site, an employee elevated on a wooden pallet by the forks of a JCB Forklift to perform welding operations was exposed to a fall hazard of approximately 11 feet.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On or about July 12, 2013, on the east side of the construction site, employees were engaged in welding operation on a commercial building from a man-lift without fall protection. The employees were exposed to fall hazards of approximately 11 feet.
