The employer exposed employees to fall hazards while mounting and dismounting an aluminum extension ladder. The side rails of the ladder did not extend at least 3-feet above the upper landing, a steep pitched, 8 in 12, unprotected roof, which was 19-feet 6-inches above ground level.
