29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about July 11, 2013, the employer did not ensure employees running joists on the roof of a building were in use of fall protection exposing the employees to a fall hazard of more than 6 feet to the ground below.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about July 11, 2013, the employer did not ensure a ladder was provided at the areas where the employees were accessing and exiting the roof of the building exposing the employees to a fall hazard.
