29 CFR 1910.1030(g)(2)(vii): The employer's training program did not contain the minimum elements required by 29 CFR 1910.1030(g)(2)(vii)(A) through (g)(2)(vii)(N):    a) Laundry Department - On or about 7/10/13, the employer's training program did not include the following elements as required by this section:        1) an accessible copy of the regulatory text of this standard and an explanation of its contents;      2) a general explanation of the epidemiology and symptoms of bloodborne diseases;      3) an explanation of the employer's exposure control plan and the means by which the employee can obtain a copy of          the written plan;      4) an explanation of the use and limitations of methods that will prevent or reduce exposure including appropriate           engineering controls, work practices, and personal protective equipment;      5) Information on the hepatitis B vaccination, including information on its efficacy, safety, method of          administration, and the benefits of being vaccinated;      6) information on the appropriate actions to take and persons to contact in an emergency involving blood or other          potentially infectious materials;      7) an explanation of the procedure to follow if an exposure incident occurs, including the method of reporting the          incident and the medical follow-up that will be made available;      8) information on the post-exposure evaluation and follow-up that the employer is required to provide for the          employee following an exposure incident. 29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) On or about 7/10/13, the employer did not develop and implement a written hazard communication program for employees who work daily with BETCO Built Detergent (containiing sodium hydroxide), Destainer 300 (containing sodium hypochlorite), Sour & Softener 440 (containing phosphoric acid), and Pro G (containing propanol).  The employer had a document which explained how to develop and implement a written program, but did not have their own program.     A written program should include descriptions of how the criteria for:        1.   Labeling and other forms of warning;        2.   Material Safety Data Sheets AND        3.   Employee information and training will be met.  Additionally, a list of hazardous chemicals known to be present in the workplace must be compiled.  Methods used to inform employees of the hazards associated with non-routine tasks and the informing of contractors of workplace hazard must also be addressed. 29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    a) Laundry Department - On or about 7/10/13, employees working with materials such as, but not limited to, BETCO Built Detergent (containing sodium hydroxide), Destainer 300 (containing sodium hypochlorite), Sour & Softener 440 (containing phosphoric acid), and Pro G (containing propanol) were not provided with information and training as required.    Employees were not informed of:        1) the requirements of this section; and      2) the location and availability of the written Hazard Communication Program, list of hazardous chemicals, and Material           Safety Data Sheets.
