29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating parts and ingoing nip points:     (a)    Vie De France Yamazaki, Inc. at 5060 Nome Street, Denver, CO 80239: On and before 7/11/13, the employer did not protect operators and other employees from hazards created by rotating parts of the "rounder machine." The cone of the rounder machine was not adequately guarded to prevent employee contact with the rotating part. This condition exposed employees to amputation hazard.     (b)   The employer did not ensure that the out feed side on the conveyor was guarded to prevent contact with "nip" points. Machine guarding or other protective measures were not in place to prevent employee incidental contact between the conveyor and the feed roller table. This condition exposed employees to an amputation hazard.     (c)   The employer did not ensure that the motor for the Oshikiri equipment was guarded to prevent contact with "rotating" parts. The cover was not on the equipment to protect employees.

29 CFR 1910.263(l)(9)(ii): All safety devices on ovens shall be inspected at intervals of not less than twice a month by an especially appointed, properly instructed bakery employee, and not less than once a year by representatives of the oven manufacturers:   a)      Vie De France Yamazaki, Inc. at 5060 Nome Street, Denver, CO 80239: On and before 7/11/13, the employer did not ensure that all safety devices on ovens were inspected annually by a manufacturer representative. The employer owns and operates 2 PONS ovens, 1 Dalhen oven, and 1 Revent oven which are natural gas fired ovens. The ovens were not annually inspected for safe operation. This condition exposed employees to carbon monoxide.   Abatement Note: Consult the oven manufacturer to determine proper inspection practices for this unique type of oven. In addition, refer to NFPA 86, "Standard for Ovens and Furnaces" for general requirements for safe oven operation.   Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
