29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   (a)  On or about 07/15/2013, 1125 Five Mile Road, Birmingham, AL, a safe means of access or egress was not provided for employees entering and exiting the excavation which was 7 feet deep.   SECOND REPEAT  The Jones Utility Contracting Co., Inc., was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.651(c)(2), which was contained in OSHA inspection number 316434133, citation number 2, item number 2, and was affirmed as a final order on 09/26/2012, with respect to a workplace located at 115 34th Street North, Pell City, AL 35125.

(a)  On or about 07/15/13 - at 1125 Five Mile Road, Birmingham, Alabama, employees were allowed to work in an excavation without protection from a possible cave-in.  The Jones Utility Contracting Co., Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.651(k)(2), which was contained in OSHA inspection number 316434133, citation number 1, item number 1b, and was affirmed as a final order on 09/26/2012, with respect to a workplace located at 115 34th Street North, Pell City, AL  35125.
