29 CFR 1910.141(a)(5): Enclosed workplaces were not so constructed, equipped, and maintained as to prevent the entrance or harborage of rodents, insects, or other vermin:  On or about July 16, 2013, the sub-basement and boot pit locations of the mill were not maintained in such a manner as to prevent the harborage of flies and maggots.  Employees working and cleaning in the areas were exposed to unsanitary conditions.

29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening:      Instance a)  The platform from the sub-basement to the boot pit where employees accessed the ladder to ascend to the boot pit was not adequately equipped with guardrails and toeboards on all open sides, and the opening was not equipped with a swinging gate or offset, exposing employees to fall hazards of approximately 6.5 feet.    Instance b)  The alternate ladderway floor opening from the sub-basement to the boot pit was not equipped with a guardrail or toeboard (except at the entrance to the ladder), and the opening was not equipped with a swinging gate or offset, exposing employees to fall hazards of approximately 6.5 feet.

29 CFR 1910.27(b)(7)(i): Metal ladders and appurtenances were not painted or otherwise treated to resist corrosion and rusting where location demands. Ladders formed by individual metal rungs embedded in concrete, which serve as access to pits and to other areas under floors, are frequently located in an atmosphere that causes corrosion and rusting. To increase rung life in such atmospheres, individual metal rungs did not have a minimum diameter of 1 inch or were not painted or otherwise treated to resist corrosion and rusting.     The fixed metal ladder from the sub-basement to the boot pit did not have rungs of a minimum diameter of 1 inch and were not painted or treated to protect against corrosion and rusting, posing fall hazards to employees using the ladder.
