29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: (a)(LOCATION) 415 Main Street, Wakefield MA 01880 On or about 4/17/2013, employees were not protected from falling while working on a flat roof that was 21 feet above a lower level. The safety monitor was engaged in other work activities that distracted his attention from the monitor function.
