29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) On and before 7/17/13, employees had not been trained to recognize and correct unsafe conditions relating to roofing work to include the proper PPE, the proper use of personal fall arrest systems, and the proper use of ladders.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On 7/17/13, employees were using an extension ladder to access the roof when the ladder did not extend at least 3 feet above the roof surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On 7/17/13, employees of Blue Planet Construction LLC, Inc were reroofing a ranch house without proper fall protection.  a) Fall protection anchors were not secured to the building correctly.  b) Two lifelines were connected to one anchor.  c) Employees had enough slack in the lifelines to be able to hit the ground.
