29 CFR 1910.157(g)(1): An educational program was not provided for all employees to familiarize them with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting:      a)  Skyline Steel - Employees utilize combustible aerosol enamel paints out on the welding floor for marking steel tubes and the employer did not provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage firefighting on or about July 11, 2013.      No abatement certification or documentation required.

29 CFR 1910.243(c)(4): The angular exposure on the grinding wheel periphery and sides for safety guard(s) used on portable grinding machine(s) exceeded 180 degrees:      a)  Skyline Steel - On or about July 11, 2013, employees were exposed to unguarded Dewalt portable hand held abrasive grinding wheels, exposing employees to potential lacerations and/or eye and face injuries from flying particles.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.252(b)(4)(iv):  The employer did not provide a means for quickly removing employees in emergency situations while performing welding operations in confined spaces and did not provide an attendant with a preplanned rescue procedure stationed outside the confined space to observe the welder at all times and capable of putting the rescue operation in effect:       a)  Skyline Steel - The employer did not provide a means for quickly removing employees in emergency situations while employees were performing welding operations inside 150 foot steel tube and did not have an attendant stationed outside the tube with a preplanned rescue procedure on or about August 7, 2013.        Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:.  a)  Skyline Steel - Employees who voluntarily wear disposable P95 respirators to protect themselves from grinding dust and welding fume were not trained on the limitations and maintenance, or at a minimum provided with that information contained in Appendix D of the standard on or about July 11, 2013.
