Thomas Tree Care Professionals, 614 South Walnut Street, Sharpsville, PA  16150, on or about July 11, 2013 and times prior thereto:  An employee was performing arborist work from the bucket of a boom truck (Simon-Telelect Hi-Ranger XT55).  Employee was not utilizing a fall protection device and fell approximately 50-feet from boom trucks bucket to the ground below.

Thomas Tree Care Professionals, 614 South Walnut Street, Sharpsville, PA  16150, on or about July 11, 2013 and times prior thereto:  Employees were performing arborist ground duties, including, but not limited to, acting as rope anchors, cleaning up tree debris, and operating the chipper.  Employees were not wearing head protection when potentially exposed to falling objects.  Potential sources of falling objects included but were not limited to falling tree debris.

Thomas Tree Care Professionals, 614 South Walnut Street, Sharpsville, PA  16150, on or about July 11, 2013 and times prior thereto:  Employees conducted arborists work and related work activities that necessitated the use of personal protective equipment.  The personal protective equipment include but was not limited to steel toed boots, gloves, hard hats, and safety glasses.  Employees did not receive personal protective equipment training on items that included but were not limited to (1) When PPE is necessary, (2) How to properly don, doff, adjust, and wear PPE, (3) The limitations of the PPE, and (4) The proper care, maintenance, useful life and disposal of the PPE.
