29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    On or about July 16, 2013, the employer did not ensure construction debris was cleared from work areas. Employees performing decking activities were exposed to slip and trip hazards while walking about the site.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    On or about July 16, 2013, the employer did not ensure electrical flexible cords were equipped with grounding pins while employees performing decking activities. The employees using the flexible cord to energize an electric air compressor were exposed to electrical shock hazards.
