29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about July 16, 2013, at a jobsite located at 5226 Vine Street workers were engaged in trenching activities installing two manholes and 66.0 feet of storm water piping system. A proper means of access and egress was not located on the west end of the trench.

Worker(s) were exposed to potential 8.5 and 9.0 foot cave-in hazards in that a proper jobsite inspection by a competent person was not conducted. .
