29 CFR 1910.217(e)(1)(i): A program of periodic and regular inspections of mechanical power press(es) was not established and followed to ensure that all parts, auxiliary equipment and safeguards were in a safe operating condition and adjustment: A)On or about July 12, 2013, in the Press Room, on the Minster Press #307, Serial Number T1326, a partial revolution press, the Die identified as Tool # T1326, was noted that one of its four guide posts was broken. The machine was operated with the damaged die for the remainder of the evening shift.
