29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     a) Employees were exposed to the hazard of falling approximately 22' from the upper roof of an industrial building to the ground during the performance of mechanical and related work activities.     b) Employees were exposed to the hazard of falling approximately 7'6" from the upper roof of an industrial building to a lower roof during the performance of mechanical and related work activities.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(3):  Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems:       a) Employees were exposed to the hazard of falling approximately 14 feet to the ground while hoisting materials and working in a hoist area at the edge of a roof deck.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:     a) Employees were exposed to the hazard of falling approximately 7'6" while climbing on a stepladder without its metal spreaders or locking devices in the open position.   b) Employees were exposed to the hazard of falling approximately 7'6" while using a stepladder for access and egress between two points; a lower roof deck, and an upper roof deck.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
