29 CFR 1910.26(c)(3)(vii): Portable ladders were used for other uses than that for which they were intended:    On or about 7/17/13, at the Dawsonville Disney Outlet store job site, an employee was using a portable metal step ladder without spreading the front and back sections of the ladder open and to locked position and instead propped the ladder against a wall for elevated work.      No abatement certification or documentation is required for this item.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i).  On or about 7/17/2013, at the Dawsonville Disney Outlet store job site, employees removed dead fronts from 120/208 volt electrical panels without providing a means of protection against the live parts located within.   No abatement certification or documentation is required for this item.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  On or about 7/17/2013, the employer did not establish a Lockout/Tagout program for employees servicing electrical equipment at the Dawsonville Disney Outlet store job site. Employees were installing circuits to a live 120/208 volt panel.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
