On or about July 18, 2013 - at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, approximately twenty (20) feet above the ground, was exposed to injuries from falls due to the lack of guardrails.  b.  On or about July 18, 2013 - at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold lookout bracket, approximately nineteen (19) feet above the ground, was exposed to injuries from falls due to lack of side rails.

LCS CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 92501, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 24, 2011 WITH RESPECT TO A WORKPLACE LOCATED AT 1758 N. HONORE STREET, CHICAGO, IL 60647
