29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On or about July 18, 2013 - at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold, approximately twenty (20) feet above the ground, was exposed to injuries from falls due to the lack of guardrails. b. On or about July 18, 2013 - at the above addressed jobsite, an employee performing masonry operations from a tubular welded frame scaffold lookout bracket, approximately nineteen (19) feet above the ground, was exposed to injuries from falls due to lack of side rails.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a. On or about July 18, 2013 - at the above addressed jobsite, an employee was performing masonry operations from a tubular welded frame scaffold which did not have the working platforms fully planked, thereby exposing the employee to falls. LCS CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 92501, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 24, 2011 WITH RESPECT TO A WORKPLACE LOCATED AT 1758 N. HONORE STREET, CHICAGO, IL 60647
