29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a. Area A building - fall protection was not used during truss operations and exposed employees to a 10'-14' fall hazard.  Violation observed on or about 7/17/13.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  a. Area A scaffold north wall - during truss operations, employee(s) were exposed up to a 6' fall hazard, in that, crossbraces were used as a means for access on to the scaffold and a ladder was not used.  Violation observed on or about 7/17/13.
