29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides and railings were not provided with a toeboard where required:  (a) Warehouse - Mezzanine areas located throughout the warehosue were not equipped with a standard guardrail system to prevent materials and employees from falling more than 12 feet to the ground.   (b) Retail Store - Mezzanine area used to collect water leaking from the roof is not provided with a standard guardrail system to prevent employees from falling more than 10 feet when retrieving buckets filled with water.

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material:  (a) Warehouse - The sides of the lower exposed portion of the blades on the Tops Power Radial Arm saw # 55508, were not equipped with a guard.  (b) Warehouse - The sides of the lower exposed portion of the blades on the Tops Power Radial Arm saw # 71744, was being help up with a bungee cord not allowing the guard to automatically adjust itself to the thickness of the blade.
