29 CFR 1910.184(f)(1): Wire rope sling(s) were used without having permanently affixed and legible identification markings as prescribed by the manufacturer, and did not indicate the recommended safe working load for the type(s) of hitch(es) used, the angle upon which it is based, and the number of legs if more than one: (a) A 5/8 inch wire rope cable sling was found hanging on the south wall of the plant. The sling did not have permanently affixed marking indicating the safe working load for the types of hitch used, the angle upon which it was based, and the number of legs, if more than one.

29 CFR 1910.304(g)(6)(vi): Exposed noncurrent-carrying metal parts of cord- and plug-connected equipment that could have become energized were not grounded: (a) A portable hand lamp which had a metal shade and was powered by only a two conductor power cord, was found in use in the grinding department on the east side of the plant. The ungrounded hand lamp exposed employees to an electrical shock hazard.
