29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system:    The employer is failing to protect employees from a fall hazard of six feet or more while engaged in rooifng activities on  low-slope roofs with unprotected sides and edges.  The most recent instance occurred at 1215 Harney St. Omaha, NE 68102, where an employee was performing roofing work near the unprotected roof edge and was fatally injured after falling greater than six feet, due to a lack of protection from falling.    The JMA Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard JMA Roofing, which was contained in OSHA inspection number 314054842, citation number 1, item number 3 and was affirmed as a final order on September 9, 2010, with respect to a workplace located at 6022 Maple St. Omaha, NE 68104.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    The employer is failing to certify that employees were trained in accordance with this subpart on the recognition of fall hazards in the work area.  The most recent instance was documented at 1215 Harney Street., Omaha, NE 68102, where an employee was performing roofing work near the unprotected roof edge and was fatally injured after falling greater than six feet.
