The most recent instance occurred at 1215 Harney St. Omaha, NE 68102, where an employee was performing roofing work near the unprotected roof edge and was fatally injured after falling greater than six feet, due to a lack of protection from falling.    The JMA Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard JMA Roofing, which was contained in OSHA inspection number 314054842, citation number 1, item number 3 and was affirmed as a final order on September 9, 2010, with respect to a workplace located at 6022 Maple St. Omaha, NE 68104.
