29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): (a) East and south side of residence, the employer exposed employees to fall hazards ranging approximately from 13-feet, 5-inches to 21-feet, 11-inches while installing a fiberglass asphalt roofing system without fall protection. The employees were working from a steep unprotected roof, 12 in 12 slope, without fall protection.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: (a) The employer did not instruct employees to recognize and avoid the hazard of falling from heights while using extension ladders. Employees were exposed to a fall hazard while improperly using extension ladders to access the roof of a 2-story single family residence. The side rails of the ladder did not extend at least 3-feet above the upper landing.
