On or about July 18, 2013 the employer did not ensure that the walking and working surface on the west side of the grill and food prep station was maintained in, so far as possible, a dry condition where there was water on the floor and an ongoing leak from the ceiling above.

On or about July 18, 2013 a floor drain opening measuring 4-7/8 inches in diameter located adjacent to the entrances for the walk-in cooler and freezer units was not effectively guarded.  b.  On or about July 18, 2013 a floor drain opening measuring 4-7/8 inches in diameter at the north end of the grill and food preparation station was not effectively guarded.  c.  On or about July 18, 2013 a floor drain opening measuring 4-7/8 inches in diameter located on the east side of the food preparation station was not effectively guarded.

On or about July 18, 2013 two lengths of electrical conductor in the attic on the east side of the attic access were joined without a splicing device leaving exposed, energized conductors where an employee was required by the employer to access the roof in response to water leaking into the food preparation area.
