1926.405(g)(2)(iii)  Flexible electrical cords were not used in continuous lengths without splice or tap:  Employees performing a residential roofing job at 745 SW Grandview in Topeka, Kansas were exposed to electrical shock hazards in that a flexible electric drop cord was used as a tap and did not retain its insulation and outer sheath properties.  The electric drop cord was modified and used with jumper clips to source power from the central air conditioner service box of the resident.

1926.451(e)(5)(i)   Ramps and walkways 6 feet (1.8 m) or more above lower levels shall have guardrail systems which comply with subpart M of this part -- Fall Protection:   Employees performing a roofing job at a two story residence located at 745 SW Grandview in Topeka, Kansas were exposed to fall hazards.  Employees were measuring and cutting four-by-eight foot sheeting in the back of a scissor lift truck.  The sheeting was carried across a ramp that served as a walkway from the truck to the roof.  The walkway was open on both sides and did not have a guardrail system in place.  The truck was extended approximately nineteen (19) feet above the ground.  The peak of the roof was approximately twenty (20) feet from the ground.

The employees were applying roof sheeting on the 8x12 pitch roof with no fall protection in place.

The ladder used to access the roof did not extended 3 feet above the roof line to allow employees safe access to the roof surface.
