29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On 7/19/13, employees of MSLT Roofing LLC were re-roofing a 1.5 story steep pitch house without appropriate fall protection. a) One employee was working without a harness. b) One employee was wearing a harness without the leg straps secured. c) One employee was connected to a lifeline that was nailed to the roof. d) Five employees did not have lifelines available on the roof. MSLT Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA Inspection Number 313136491, Citation Number 1, Item Number 1 and was affirmed as a Final Order on 3/29/12, with respect to a workplace located at 8435 Elderberry Ln, Madison, WI 53717. MSLT Roofing LLC was previously cited for a violation of this Occupational Safety and Health Standard, which was contained in OSHA Inspection Number 313130502, Citation Number 1, Item Number 1, and was affirmed as a Final Order on 1/14/11, with respect to a workplace located at W9109 Rabbit Run, Cambridge, WI 54523.
