29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Bathroom in Data Wipe Area Employees directed to renovate a bathroom were not provided with information and training to recognize potential hazards or unsafe conditions during the project. Potential hazards included, but not limited to, live electrical wires from water heater, air dryer & electrical outlets/switches. Violation occurred on or about 7/19/13.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Bathroom Employees removed protective enclosures from the bathroom outlet, light switch, and water heater during painting preparation. The equipment was not deenergized and grounded nor was it guarded effectively by insulation or other means. Violation occurred on or about 7/19/2013.
