29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) 3rd ave. - two employees were exposed to a fall of 10 feet 3 inches while standing on beam forms which were on a flat bed truck. The employees were rigging the forms, on or about 7/11/13.
