29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about 07/16/2013 at 220 Tom Hill Sr. Blvd., Macon, GA: Two employees were working on the roof of a gasoline pumping station were exposed to a fall hazard of approximately 18 to the lower level. To access the roof the employees used a 25-foot extension ladder that did not extend 3 feet above the ledge for a means of safe access/egress.

29 CFR 1926.1053(b)(21): Each employee was not grasping the ladder with at least one hand when progressing up and/or down the ladder. On or about 07/16/2013 at 220 Tom Hill Sr. Blvd., Macon, GA: An employee working on the roof of a gasoline pumping station was exposed to a fall hazard of approximately 18 feet to the lower level when he was observed descending an extension ladder carring multiple spools of wiring.
