29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 192 Lee Rd 2137 Phenix City, Alabama: On or about July 17, 2013 and at times prior; the employer failed to ensure a portable extension ladder used on site by employees was extended at least three feet above the landing area exposing them to fall hazards of 19.7 feet to the ground below.
