The exposed employees were not protected by any type of fall protection system, on or about June 18th 2013.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: A) Front upper level of house - Employees were exposed to fall hazards of approximately 19 feet while using an extension ladder to access the upper roof landing. The ladder that was used, did not extend at least 3 feet above the landing, on or about June 18th, 2013.
