Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. a.) Lamb & Robinson; Naval Station Norfolk, Barracks KK: On or about 07/16/13, extension cords were not being used in accordance with listing and labeling requirements in that two 50' cords and one three way pigtail cord were connected together to supply power to a personal fan thereby potentially exposed employees to a fire and/or electrical shock hazard.

Lamb & Robinson; Naval Station Norfolk Barracks KK: On or about 07/16/13, the wheels of a mobile scaffold did not lock while employee was working 3' from the ground thereby potentially exposed employee to a fall.
