29 CFR 1926.501(b)(13):   Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Worksite located at 16622 Oak Circle, Omaha, NE.  This is a residential site:  Employees were observed conducting construction activities on a residential structure.  The employee and owner were exposed falls of 7 feet or more without using any form of fall protection such as a personal fall arrest system.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.503(a)(1):   The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     Throughout the company and the Worksite located at 16622 Oak Circle, Omaha, NE:  The employer failed to train employees that were potentially exposed to fall hazards to enable each employee to recognize the hazards associated with the type of work being conducted.  This should include training on mitigating those hazards through an acceptable method of fall protection.  Training should also include the requirements for fall protection systems and when it is appropriate to utilize those systems.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(4):   Ladder(s) were used for purposes other than the purpose for which they were designed   Work site located at 1662 Oak Circle, Omaha, NE.  This is a residential site:  The top section of an extension ladder was being utilized to access a roof surface.  The ladder is not designed to be used in separate sections as outlined in the labeling on the ladder side rail.       Abatement certification is required for this violation.
