Throughout Facility - On or about July 19, 2013, the employer had not performed annual periodic inspections to ensure that documented lockout/tagout procedures were being followed by authorized employees.

29 CFR 1910.147(c)(7)(i):   The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees.  The training shall include the following:  Throughout facility - On or about July 19, 2013, the employer did not provide effective training to authorized, affected, and other employees to include all of the elements in (a) through (c), thereby exposing employees to unexpected re-energization of equipment.

29 CFR 1910.147(d)(4)(ii):  Lockout devices, where used, shall be affixed in a manner to that will hold the energy isolating devices in a "safe" or "off" position.  Processing Department- On or about July 19, 2013, the employer did not ensure that lockout devices were used to isolate all energy sources, thereby exposing employees to potential re-energization of equipment.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.  Throughout facility - On or about July 19, 2013, while repairing and replacing parts on presses and other operating equipment, all employees did not perform group lockout/tagout, thereby exposing employees to crushing type injuries.  The first pressmen or maintenance are the only employees that locked out the equipment while other employees helped perform repairs.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Throughout Facility - On or about July 19, 2013, the interlock used as guarding for the Gammerler KL 6000, was inoperable, thereby exposing employees to the point of operation.
