29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: This violation occurred on July 22, 2013, at a workplace located at 5016 Province Place, Irving, TX 75063; where at least four (4) employees, who were working at various elevations under construction activities, were not protected from head injuries by wearing hard hats. The Gerardo Renteria Dba GRM Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a) which was contained in OSHA inspection number 314184599, citation number 1, item number 1 and was affirmed as a final order on 05/10/2012, with respect to a workplace located at 1226 Wedgewood Dr., Forney, TX 75126.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: This violation occurred on July 22, 2013, at a workplace located at 5016 Province Place, Irving, TX 75063; where at least four (4) employees, who were working from rafters of a two-story house, were not protected from falling to the ground below. The Gerardo Renteria Dba GRM Framing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 314181561, citation number 1, item number 3 and was affirmed as a final order on 05/30/2012, with respect to a workplace located at 700 Osage Drive, McKinney, Texas 75069 ; and which was contained in OSHA number 312125834, citation number 1, item number 5, and was affirmed as final order on 03/09/2010, with respect to a workplace located at 1201 Grayhawk, Forney, Texas 75126; and which was contained in OSHA number 314176546, citation number 2, item number 4, and was affirmed as a final order on 06/02/2010, with respect to a workplace located at 898 Honey Locust Drive, Fate, Texas 75132.
