29 CFR 1928.110(c)(2)(ii): Toilet facilities were not adequately ventilated, appropriately screened, capable of being latched from the inside with a self-closing door, or constructed to insure privacy: (a) Kula Agriculture Park, Lot #11: On July 15, 2013, and at times prior thereto, the toilet facility on site did not have a door and was missing a portion of the roof. The entrance platform to the toilet facility had three missing or broken boards.

(a) Kula Agriculture Park, Lot #11: On July 15, 2013, and at times prior thereto, the toilet facility contained a dirty, rusted sink and the toilet contained colored water that had not been flushed or cleaned. The area around the toilet facility was cluttered and posed a trip and fall hazard.
