29 CFR 1926.1425(c)(1): 29 CFR 1926.1425(c)(1): The employer did not make sure that the materials being hoisted when employees were hooking, unhooking, or guiding the load were rigged to prevent unintentional displacement. a) Wanamaker Plaza, West Tower, 11th floor - On or about 7/02/13, the employer did not ensure hoisted materials were rigged to prevent unintentional displacement putting employees guiding loads at risk of injury. A load in one instance guided by employees pitched violently, slipped out of the rigging, fell through the 14th level, and broke through the 13th and 12th floors before landing on the 11th floor.
