29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     At the time of inspection, the employer did not establish a program to protect employees involved with servicing and maintenance of machines and equipment from the unexpected energizing, start up, or release of stored energy.  Energy sources included, but are not limited to, electrical, pneumatic, hydraulic, etc.  The company maintains and services equipment such as, but not limited to, CNC machines, hydraulic presses, grinders, etc.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     a) At the time of inspection, the Makio CNC machine doors were not interlocked to prevent them from being opened during production or running of equipment.     b)  At the time of inspection, the Hitatchi Seiki vertical machining cell doors were not interlocked to prevent them from being opened during production or running of equipment.     c) At the time of inspection, the VS50 vertical machining cell doors were not interlocked to prevent them from being opened during production or running of equipment.     d) At the time of inspection, the Mazatrol Turn-Smart 250 CNC lathe doors were not interlocked to prevent them from being opened during production or running of equipment.     e) At the time of inspection, the Mazatrol "T" plus CNC lathe cell doors were not interlocked to prevent them from being opened during production or running of equipment.     f) At the time of inspection, the Hitatchi Seiki 40G CNC machine cell doors were not interlocked to prevent them from being opened during production or running of equipment.     g) At the time of inspection, the QT20 lathe cell doors were not interlocked to prevent them from being opened during production or running of equipment.     h) At the time of inspection, the Mori-Seiki super hi-cell 250 cell doors were not interlocked to prevent them from being opened during production or running of equipment.     i) At the time of inspection, the LNS quick load servo S2 parts loader contained an interlock device to prevent the opening of the gate during feeding of the parts. This interlock was bypassed or not connected to allow the loader to operate with the gate open.     j) At the time of inspection, the LNS quick load servo S3 parts loader contained an interlock device to prevent the opening of the gate during feeding of the parts. This interlock was bypassed or not connected to allow the loader to operate with the gate open.     k) At the time of inspection, the NL2500 parts loader contained an interlock device to prevent the opening of the gate during feeding of the parts. This interlock was bypassed or not connected to allow the loader to operate with the gate open.     l) At the time of inspection, the Cincinnati Centerless grinder did not contain guards on both sides of the wheel where point of operation hazards existed.
