Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Worksite: On or about 07/23/2013 an employee fell 75 feet through an opening that had an unsecured cover on the scaffolding system.

29 CFR 1926.503(c): "Retraining." When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee. Circumstances where retraining is required include, but are not limited to, situations where: Worksite: Employees were not retrained to recognize hazards in which they were exposed to while working from a scaffold suspended on the underside of the Mount Hope Bridge in Bristol, Rhode Island. Consequently on or about 07/23/2013 an employee fell approximately 75 feet to the ground below and sustained fatal injuries.
