29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which were used by employees did not have approved ground fault circuit interrupters for personnel protection: Employees using power tools connected to an extension cord with two cuts in the insulation were exposed to electrical shock and the employer did not ensure the temporary wiring was connected to ground fault circuit interrupters.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, was visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage: Employees using power tools connected to an extension cord were exposed to electrical shock and the employer did not ensure the cord set was visually inspected before each days use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.
