29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift.  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:     (a)  Employees operated an aerial lift without any form of fall protection. 29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:     (a)  Employees engaged in roofing activities on a barn with a roof slope ranging from approximately 4:12 to near vertical did not utilize any form of fall protection to be protected against falls of greater than 6 feet. 29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a)  Employees were not provided a training program to recognize fall hazards and the procedures to be followed in order to minimize the hazards.  On or about July 22, 2013, employees demonstrated the lack of a training program by engaging in roofing activities at a height of greater than 6' without utilizing any form of fall protection.          ABATEMENT CERTIFICATION AND ABATEMENT REQUIRED
