29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Warehouse: A steel beam attachment with a safety hook and lifting clamps was added to the Hyster Forklift truck to lift slabs of granite and was not approved by the manufacturer prior to use. Bargain Tile Surplus LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 178(a)(4) which was contained in OSHA inspection number 315974311, citation number 1, item number 4, issued on March 21, 2012, with respect to a workplace located at 55 Connair Rd, Orange, CT 06477 and became a final order on April 6, 2012.
