The workers were exposed to an approximate 18-21 foot fall hazard as they worked on the roof without the use of an adequate fall protection system and/or fall arrest system.  In addition, workers were exposed to an approximate 18 foot fall hazard as they used an extension ladder that did not extend 36 inches above the landing while carrying full sheets of plywood preventing three-point contact with the ladder.
