29 CFR 1910.23(a)(5):     Every pit and trapdoor floor opening, infrequently used, shall be guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings.    Worksite located at 500 1st Terrace, Nebraska City, NE; Railcar Load Out Area:  A hatchway in the catwalk system at the railcar load out structure is not protected using a system as allowed by the standard.  In this case, when the hatch is lowered the floor hole is covered.  When the hatch (trapdoor) is opened, there is a fall hazard of greater than 20 feet.       Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    Worksite located at 500 1st Terrace, Nebraska City, NE; Basement of the Silo:  The employer failed to ensure that all open-sided floors and/or platforms four feet or more above the adjacent floor or ground were guarded with standard railings as defined by the standard.  An area at the edge of the basement floor/platform has an opening which is 17 X 31 inches and a person could partially or completely fall through the opening.  The opening is adjacent to the walkway for access to one of the under-silo conveyors.       Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1910.24(b):     Fixed stairs were not provided for access from one structure level to another where operations necessitated regular travel between levels, and for access to operating platforms at any equipment which requires attention routinely during operations:     Worksite located at 500 1st Terrace, Nebraska City, NE; Basement of the Elevator:  The employer has failed to install a fixed industrial stair system for access to the basement of the elevator.  Currently there is a spiral staircase which is used as the primary (only) access method for the area.  The standard does not allow for the use of spiral stairways as the primary route of access/egress.         Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

Some of the procedures were reviewed by a two person team and others were reviewed by a single party.        Abatement certification is required for this violation.
