29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. At this job site, employees did not have a ladder or ramp or other safe means of egress from a trench greater than 4 feet in depth and greater than 25 feet long. Employees were stepping up onto a 30 inch diameter pipe from the trench floor and then jumping up to a bench for egress from the work area. Saenz Utility Contractors, LTD., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 312386519, citation number 1, item number 1 and was affirmed as a final order on November 18, 2008, with respect to a workplace located at Bouldin and FM 508, Combes, TX 78535. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). At this storm drainage project employees entered and worked in a trench greater than 5 feet in depth without an adequate employee protective system. The trench was not prepared with sloping or adequate bench, shoring, or a trench shield. Saenz Utility Contractors, LTD., was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 312386519, citation number 1, item number 3, and was affirmed as a final order on November 18, 2008, with respect to a workplace located atBouldin and FM 508, Combes, TX 78535.
