A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.  At this job site, employees did not have a ladder or ramp or other safe means of egress from a trench greater than 4 feet in depth and greater than 25 feet long. Employees were stepping up onto  a 30 inch diameter pipe from the trench floor and then jumping up to a bench for egress from the work area.
