29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): The employer does not ensure that procedures clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy. This violation occurred on or about July 23, 2013, in the front fabrication shop where CNC operators, programmers, and maintenance employees were exposed to struck-by and caught-in hazards when performing set-up or maintenance tasks on lathes such as but not limited to, the CNC HASS ST-20 and DaeWoo vertical lathe without specific procedures to control hazardous energy. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that procedures clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not guard the rotating parts of machines whose operation exposes an employee to injury. a) This violation was observed on or about July 23, 2013, in the fabrication shop where employees were exposed to struck-by hazards when operating two Enco engine lathes without chuck guards. b) This violation was observed on or about July 23, 2013, in the fabrication shop where employees were exposed to a struck-by hazard when operating a Harrison engine lathe without a chuck guard. c). This violation occurred on or about July 23, 2013, in the fabrication shop where employees were exposed to a struck-by hazard when operating a Bullard 42" vertical lathe without a chuck guard. d) This violation was observed on or about July 23, 2013, in the fabrication shop where employees were exposed to a struck-by hazard when operating a Summit engine lathe without a chuck guard. e) This violation occurred on or about July 23, 2013, in the fabrication shop where employees were exposed to a struck-by hazard when operating a King vertical lathe without a chuck guard. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all machines that expose employees to hazards are guarded.
