29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about July 9, 2013 employees of Kelley Masonry Inc. Were exposed to electrical hazards while working with a saw that had exposed electrical wires due to a lack of strain relief.

ON or about July 9, 2013 employees of Kelly Masonry Inc. were exposed to caught-in, caught-between, and fall hazards due to scaffolds not being braced.
