Location: 85-21 168 Street Queens, NY.     On or about: 07/22/13     a) Employees were working next to an unguarded area without fall any protection system while exposed to a potential fall of approximately 8 feet to the ground below.

On or about: 07/22/13  a) Employees were using a scaffold plank as a runway to acces to the back door of the house, no guardrails were provided, the employees were exposed to a potential fall of approximately 6 feet to the ground below.

Location: 85-21 168 Street Queens, NY.     On or about: 07/22/13    a) Employer did not provide a stairway, ladder or runway at the point of access. Employees were using a window to access the work place.

On or about: 07/22/13    a) Employees were using a portable ladder to gain access from the basement to the first floor of the house, but did not have the ladder side rails extending 3 feet above the upper landing surface.
