29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided.       Location: 85-21 168 Street Queens, NY.     On or about: 07/22/13    a) Employer did not provide a stairway, ladder or runway at the point of access. Employees were using a window to access the work place.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge.    Location: 85-21 168 Street Queens, NY.   On or about: 07/22/13  a) Employees used the stairways throughout the project which were not equppied with stairrails and handrails along each unprotected side or edge.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.      Location: 85-21 168 Street Queens, NY.     On or about: 07/22/13    a) Employees were using a portable ladder to gain access from the basement to the first floor of the house, but did not have the ladder side rails extending 3 feet above the upper landing surface.
