29 CFR 1910.134(d)(1)(iv): The employer did not select respirators from a sufficient number of respirator models and sizes so that the respirator is acceptable to, and correctly fits, the user: (a) On or about 07/24/2013, at jobsite , for the laborer wearing a large survivair half face respirator during demolition. The employee was not provided with the right size respirator. He was fit tested for a medium survivair half face respirator.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) On or about 07/24/2013, at the job site, for the supervisor and laborer in the area of the demolition. Employees on the ground were not wearing hard hats.
