(a) Warehouse - On or about July 09, 2013, there was an accumulation of wire baskets, baskets of metals, large cans of metal and other materials on the floor thereby exposing employees to a tripping or fall hazard.

(b) Warehouse - On or about July 9, 2013 employees were using unsanitary microwave and refrigerator for food storage and consumption.
